Previous MRI, CT or ultrasound reports. Bring a CD or digital copy of the images if possible — not just the reports.
A printout of all previous PSA results with dates — the trend over time is often as informative as the most recent result.
Full pathology report with Gleason score / ISUP grade group and core involvement data.
Any correspondence from your current or previous urologist, oncologist or GP related to this condition.
